I just picked up a CB3K and am giving it its maiden run this weekend. That said, I’m running into some issues because of my testicle size. Once the base ring is on, they stay secure and forward-facing. But when the cage clips in and locks, it pushes them back quite hard against each other, which feels pretty uncomfortable. Has anyone else dealt with this? Is there a different model out there that doesn’t have such a wide base ring as the CB3K and might fit better? Another issue is getting my old-fashioned member into the cage. The foreskin tends to get dragged backward, adding to the discomfort while wearing it. Any advice would be great.

But when the cage clips in and locks, it pushes them back quite hard against each other, which feels pretty uncomfortable. You ought to have a set of pins with varying lengths along with several spacers. Use those spacers to tweak the gap between the cage and the ring, picking whichever pin is long enough for your spacer setup. Ideally, you want that gap tight enough so they don't pop through, but loose enough to avoid the discomfort you're feeling now. If you go with a longer pin leaving room at the end, add extra spacers there too; this helps keep the lock snug and holds the cage firmly against the spacer(s) separating it from the ring. Hold off on cutting or filing those guide pins until you've settled on your ideal locking pin and spacer combination. As for the foreskin issue, I can't offer much help since I'm lucky enough to not have that part.
